Immunizations, also called vaccinations, are one of the greatest achievements in public health. Vaccines prevent disease
in people who receive them. Additionally, if enough people in the community are vaccinated, the entire community can be
protected because there is little opportunity for an outbreak to occur. The American Academy of Pediatrics supports routine

Please contact your or your child’s medical provider or the Wood County Health Department for immunizations. The Wood County
Health Department holds an immunization clinic at their office in Wisconsin Rapids once a month. Restrictions
and charges may apply. To schedule an appointment for our immunization clinics, for information on cost or restrictions,

Important Notes: PARENTS OR LEGAL GUARDIAN must sign the immunization form BEFORE
a child is immunized.
Fees: We accept cash or check. We ask for a $10.00 donation per child/adult (cost
is waived for child vaccinations if MA eligible). For adult Hepatitis B vaccine the charge is $65.00 per dose.
Available Childhood Vaccines: DTaP (dipteria, tetanus and pertussis), Polio, Pediarix (DTaP, Hepatitis B, polio),
Hepatitis A and B, Pneumoccal, Rotovirus, Influenza, Hib (influenza type B), Tdap (tetanus, diptheria, pertussis), HPV, Meningococcal, MMR
(measles, mumps, rubella), Varicella (chickenpox), and Meningitis B.
Adult Vaccines: Tdap and Hepatitis B. Other vaccines are available to people who meet certain
criteria. Please call the Wood County Health Department for more information.
